Hockey Roster Template

By Riku Pelkonen•Last verified

In Massachusetts youth hockey, the roster you coach from has a printed twin, a 2-by-4-inch sticker for the game sheet. Every team puts that label on every copy of the official game sheet, and it lists each rostered player and coach. Methuen Youth Hockey(opens in new tab) counts three copies per game. If your league still keeps score on paper, ask whether it wants the same label.

This page builds two documents. The sticker builder prints your label ten times on one Avery 5163 or 5263 sheet, CEP numbers included. The season roster is the one you coach from, with a column for every detail that decides your lines. Here only for the season roster? Jump to Season Roster: Position, Shoots, and Letters.

USA Hockey's Official Rules and Casebook 2025-2029 decides what both sheets can hold. Its 20-player limit sets the line count. Its jersey rule allows whole numbers from 0 to 99, so check every number against it before you print.

What Goes on a Hockey Roster Sticker

The official Massachusetts Hockey template(opens in new tab) asks for four things. Your team code, the team name exactly as the league lists it, every rostered player, and every rostered coach. Players go in alphabetical or numerical order. Coaches go in as first name, last name, CEP number, CEP level, and the year it expires. The label measures 2 by 4 inches, which is the size of Avery 5263(opens in new tab) and Avery 5163(opens in new tab), ten labels to a sheet.

A Massachusetts-style game sheet label, with team code and team name on top, 20 player lines, and 4 coach lines.

In Massachusetts the team code looks like MAHXXXX-XXX and includes your roster number. Where your league assigns a team code, copy it from the official USA Hockey roster your program registrar submits. In Massachusetts that roster also carries the coaches' CEP numbers and levels, CEP being USA Hockey's Coaching Education Program. Never type any of it from memory.

Some names never reach the printed label. Methuen's instructions(opens in new tab) say the names of players playing up, and of coaches who will be on the bench, get handwritten onto each sticker. In Massachusetts, skipping the label altogether has a cost. Winchester Youth Hockey(opens in new tab), another Massachusetts club, warns that failing to use stickers for any game may result in a minor penalty.

How Many Lines Your League's File Holds

Your league's or club's file decides how many player and coach lines one label holds. The Massachusetts template has room for all 20 players and four coaches. Arrowhead Youth Hockey(opens in new tab) publishes its Google Sheets sticker in three layouts instead.

  • 17 players and 3 coaches
  • 16 players and 4 coaches
  • 14 players and 7 coaches

The sticker builder below follows the Massachusetts layout. If your league or club asks you to use its own file and that file comes in tighter layouts, count your rostered players and coaches and pick the layout that fits both numbers. If none has room for your whole roster, ask the league which version to use before you write anyone in by hand.

Build the Sticker: Player and Coach Lines

USA Hockey Rule 201(b) wants a Head Coach designated on the scoresheet before every game, so give the head coach the first coach line. Names print exactly as you type them, capitals included.

The label stops at 20 players. A Tier I team that dresses 22 under Rule 201(a) should ask its league which label file to use. Nothing you type here is saved, so keep your list in a spreadsheet and type the new version in when the roster changes.

Your label appears here as you type the team, players, and coaches.

In the print window, set the scale to 100% (Actual size) and turn off headers and footers. Try one sheet on plain paper first.

Type a team, a player, or a coach to turn on printing.

Printing on Avery 5163 or 5263 Labels

Check your league's format before you buy a box of labels. Nashoba Youth Hockey(opens in new tab) notes that games in one of its leagues, the MYGCL, no longer use roster sheets, because coaches manage the roster on the scorekeeper's iPad at the rink. Paper labels only matter where the game sheet is still paper.

The Print labels button in the sticker builder above puts your label on all ten spots of one Letter sheet, laid out for Avery 5163 and 5263, which share one template(opens in new tab).

  1. Keep the scale at 100%, as the note above the button says. Any shrink-to-fit setting moves every label off its sticker.
  2. Print one page on plain paper and hold it over a label sheet against a window. Nashoba notes that its own Excel label file can need small spacing adjustments for a particular printer, and this test shows you where before it costs a sheet. If every label sits off by the same amount, check the scale and the headers and footers again. If it still misses, print from Avery's template or your league's file.
  3. Print the real sheet. Check the first label against the registrar's roster before you peel anything.

A game sheet with three copies takes three stickers, so one ten-label sheet covers three games with one label to spare. Every label on a sheet is identical, so those spares count toward later games, and a 30-game season needs 90 labels, which is nine sheets, as long as the roster never changes. Each change starts a fresh batch, because the old stickers no longer match the roster your registrar holds.

The same print window can save the label sheet as a PDF, ready for the next reprint. The season roster below has its own Print Form for a full-page copy. Its Copy as Table rows paste into Excel, where you keep the number and name columns for a club label file like Winchester's template for Avery 5263(opens in new tab).

Season Roster: Position, Shoots, and Letters

This roster starts with 17 pre-filled rows: three forward lines of C, LW, and RW, three defense pairs, and two goalies. Three open rows bring it to 20, the most a team can dress under USA Hockey Rule 201. Teams in the Tier I categories the rule names can add two more.

Type over the pre-filled positions as you go. Shoots takes L or R for skaters. For goalies it records the catching hand (L = glove on the left). Pos takes C, LW, RW, D, or G, and a forward who plays two spots can carry both, as C/RW. C/A holds the letters, and Birth Year helps with registration questions and with the day a younger player plays up.

USA Hockey Rules 201 to 203, Line by Line

Three short rules in the USA Hockey Official Rules and Casebook 2025-2029(opens in new tab) decide most of what a roster and a game sheet can hold. The middle column puts each rule in plain words. The right-hand column is our reading of what it means for your paperwork, and it is not rule text.

RuleWhat the rule saysOn your sheet
201(a)A maximum of 20 players may play, and not more than 18 of them may be skaters.Twenty player lines. With only one goalie, 18 skaters plus that goalie makes 19.
201(a) noteYouth Tier I 15-Only, 16U and 18U, and Girls Tier I 19U teams may dress 22, with up to 20 skaters. Two extra lines, only if your team plays in one of those categories.
201(a)A team must start with at least six players and forfeits if reduced to fewer than four.Six dressed players is the floor for puck drop.
201(b)A Head Coach is designated on the scoresheet before the game. Up to four Team Officials may sit on the bench.Four coach lines cover the bench. Know which one is the head coach.
201(b)A rostered player who cannot play (other than through suspension) may sit on the bench in jersey and head and face protection, but is not included on the game sheet as an eligible player. An injured player's name comes off that game's eligible list. Ask your league how to mark it.
202(a)One Captain and not more than two Alternates are designated on the scoresheet before the game. Goalkeepers cannot use Captain privileges. One C and at most two A in the letter column.
203(a)The Manager or Coach designates the eligible players and goalkeepers on the scoresheet before the game. A player added after the start costs a bench minor.Settle the list before you reach the scorer's table.
203(b)Each player wears an identifying number, whole numbers 0 to 99 only.Number column: 0 through 99, no letters.
203(c)Each team is recommended to have a substitute goalkeeper, fully dressed and equipped. Two G rows on the season roster.

Rule 203(opens in new tab) attaches conditions to a late addition that the table leaves out. It can only happen at a stoppage, nobody already on the game sheet may come off to make room, and the maximum still has to hold. Your state affiliate and league may add their own rules on top.

Count the Shoots Column Before You Pair Defense

Fill in Shoots for every skater before you sketch a single line. Then count. Six defensemen who all shoot left still make three pairs, but three of them will play the right side on their off hand.

That changes what each player sees. On the natural side, a defenseman's forehand faces the boards, where rims and breakout passes arrive. On the off side, the forehand opens toward the center of the rink. Some coaches want that for shots from the point. Others want the forehand on the wall. Either way, decide it in the first weeks of the season. Moving a player across or drilling a backhand outlet pass costs nothing in practice, and league play will make the call for you if you wait.

Run the same count on your forwards. Put a right-handed shooter at left wing and the forehand opens to the net, and some staffs put scorers there on purpose. Count both wings anyway. A roster with seven left shots up front tells you early who will be playing the off wing.

Write the reasons behind each pairing, such as the player who skates better backward or reads the rush earlier, on a per-player sheet. Our hockey player profile template keeps those notes per player, right next to the shot side.

Moving Your Roster Into Striveon

In Striveon, the roster comes in through the Import players button on the Players page. On the Paste Names tab, paste one name per line and continue straight to the Preview step. On the File tab, upload the sheet as .xlsx, .xls, or .csv, then match its columns on the Map Columns step: Full Name (or First Name and Last Name), Birth Date / Year, and Group / Team. If you import during first-time setup, the Split into teams section of the setup screen offers a From file option, which uses that team column.

Jersey numbers and positions are not part of the import. Once it finishes, you open each player and add them yourself in the Position and Jersey number fields, and after that the player list searches by both. Shoots, the C and A letters, and CEP details stay on your own sheet, or in a note on the player.

If you start from ice hockey's starter content when you set up Striveon, you get these five skill sets, plus 15 drills and four practice sessions, all yours to edit.

  • Skating, with Forward Skating, Backward Skating, Turn Skating, and Transitions
  • Puck Skills, covering Stick Handling, Passing, Puck Protection, and Receiving
  • Shooting, split into Wrist Shot, Slap Shot, Snap Shot, and Shooting in Motion
  • Defensive Play, from Backchecking and Forechecking to Defensive Positioning and Battling Skills
  • Game Sense, which holds Game Reading, Decision Making, Positioning, and Shift Management

From there you can rate each rostered player against those skill sets with Striveon's athlete evaluation, score tryouts on paper with the hockey evaluation form, and plan the next skate around what you saw with the hockey practice plan. If you run several teams in one association, Striveon's training management holds every team's athletes and practice schedule side by side, while the sticker builder on this page stays what you print for the scorer's table.
